Username: eplus
Password: internet

Go to LTE Setup (Optional) section and enter information as below

APN: (Leave Blank)
Username: (Leave Blank)
Password: (Leave Blank)

Press the Home button to save the APN and exit to the main screen.
Restart the iPhone.
